This is a list of resources to help you navigate the different stages of the end-of-life journey.

Lots of support exists to help you when you need it.

The following is a list of resources to support you and your loved ones when moving towards the end of life. This is a general list of Ontario resources that will help you identify services specific to your region. If you need help finding more specific resources or more local ones, please reach out any time - I would be happy to help.

  • Caregiver Exchange - Online resource providing information and support for caregivers in Ontario.

  • Cancer Care Ontario - Palliative and End-of-Life Care - Resources and information specific to palliative and end-of-life care for cancer patients.

  • Hospice Palliative Care Ontario (HPCO) - Provides leadership and support for hospice palliative care providers across Ontario.

  • Ontario Palliative Care Network (OPCN) - Coordinates and improves access to palliative care services throughout the province.

  • Ontario Association of Community Care Access Centres (OACCAC) - Offers information on home and community care services, including palliative care.

  • Pallium Canada - Offers palliative care education and training programs for healthcare providers and volunteers.

  • VON (Victorian Order of Nurses) Canada - Offers home and community care services, including palliative care, across Ontario.

  • Saint Elizabeth Health Care - Offers home care and palliative care services across Ontario.

  • Caregiver Resource Network - Provides resources and support for caregivers in Ontario, including those caring for individuals at end-of-life.

  • Trillium Gift of Life Network - Coordinates organ and tissue donation and transplantation services in Ontario.
